How to get approved
Four steps to volunteer or drive students on field trips.

Complete the Consent Form
Fill out the online Consent Form (the self-disclosure section is optional). Paper copies are available at the Finance Office.

Schedule your fingerprinting
Once your consent form is in, we'll email your authorized paperwork. Book a Livescan appointment with Yup Fingerprinting (the Lehi location is closest) and bring a photo ID.

Submit the Field Trip Driver Form
After we notify you that your background check has cleared, complete the Field Trip Driver Form before each trip you drive.
Required Forms & Documents
Please review these before your fingerprinting appointment. The school-covered Payment Form only applies if the academy is covering your fee.
